Functional medicine is a way to approach internal medicine/ primary care–offering in depth medical consultation and functional medicine tests not always available in a traditional medical practice. While many patients will find answers to their medical concerns with just a few visits, others will require more ongoing care.
Deva Health, PLLC does not provide urgent care–you must contact your primary care physician or local emergency department for any urgent/emergent issues. Deva Health, PLLC requires that all patients have a relationship with a Primary Care Physician in their insurance network.
Comprehensive Initial Visit: $950
For all new patients, this includes: Pre-visit questionnaire/s, 2 hour comprehensive evaluation (history and physical examination), access to email support with Dr. Barry for 30 days.
Laboratory tests are not included. The need for other tests will be determined at the time of your visit (tests are processed through insurance when possible; some specialized tests are not processed through insurance).
Follow Up Visit: $300+
Rates are based on time spent with the patient ($300/45 min, $350/60 min). Most follow up visits require approximately 45 minutes.
Email Support: $250+ per month
For patients with chronic conditions who require ongoing email support after their initial 30 days. Rates based on time required for counsel. This service will most likely not be reimbursed by insurance.
Laboratory tests are based on each individual patient’s needs, and will be discussed during patient visits. Laboratory tests are priced separately from the visits listed above. Diagnostic laboratories used include Quest Diagnostics, LabCorp, Genova, IgeneX, Galaxy, Doctors Data, Boston Heart, Armin Labs.
• $100 fee for ECG (electrocardiogram, 12-lead with interpretation/report)
To complete paperwork for disability claims, workers compensation, leave of absence for work, etc. there will be a minimum fee of $35 + $5 per page, paid in advance. For Prior Authorization or contacting insurance companies, a processing a fee may be charged for time spent processing (e.g. $25+).
Payment for service is expected at the time of your visit–checks, cash and most major credit cards are accepted. We do not accept medical insurance, but you may request a superbill to submit to your insurance company for reimbursement. We recommend you inquire about your insurance plan’s policy for “out of network” providers. Lab tests ordered at Quest or LabCorp will be processed through insurance. Some specialty labs will require pre-payment at the time a test is ordered, but a paid invoice may be submitted to insurance by the patient.
